Her declaring her feelings might not have happened on screen. I always had the feeling that it did -- there were hints such as the "do you like me?" and "why did you hire me." But you might be right that we didn't see it.
As for his pushing her, that is very much a symptom of her "it's all about me" syndrome. Frequently when House disagrees with her she takes it personally -- as if she thinks that House would shoot down the correct diagnosis just so he could make fun of her.
Occam's Razor -
HOUSE: Well, that changes everything. You need glasses. That’s why you had double vision, which is why you got hit, which is why you have a concussion, which is why you have night terrors. You need to see an ophthalmologist, which I am not.
CAMERON: You enjoyed that. I brought a reasonable case to your attention, and you shoved it in my face just to humiliate me.
HOUSE: You’re an only child, aren’t you?
CAMERON: Why would you say that?
HOUSE: Because everything is about you.
TB or Not TB -
CAMERON: And it’s not glucagonoma because he has no rash. It’s not self-induced because he’s not an idiot, and it’s not a tumor because the CT and the MRI were both negative.
HOUSE: Which just leaves tumor.
CAMERON: Why do you do this? Why do you ignore what I say like I’m not even –
Your example from Heavy.
I'm sure there were other examples that I can't think of right now.

But I'm curious about the example from "Heavy." After all, House proceeded from the notion that Cameron had screwed up the administering of warfarin, which she protested. It wasn't until the end of the episode that House was willing to consider that perhaps she hadn't screwed up. I don't think that means Cameron was making it all about her more than not wanting to be thrown under the bus. At least not in that instance.

Thanks for your input, bailey and Poeia. I just found the scene with Chase. It's in Mob Rules, the episode between Control and Heavy.
Cameron: Did House seem weird to you?
Chase: Are you expecting him to be weird?
Cameron: We spoke about how we felt.
Chase: You told him you liked him?
Cameron: No, of course not.
Chase: What are you talking about, then?
Cameron: I asked him if he liked me.
Chase: Why would you do that?
Cameron: Because I like him.
Chase: You like him, like him?
Cameron: Doesn’t matter, he doesn’t like me.
Chase: Hey, he doesn’t like anybody. And nobody likes him.
So I guess you're right, that by saying "Do you ?" to House, she was round-aboutly saying that she "likes him-likes him." And House of course figured that out immediately. And that is why in Heavy he says, "I believe that you are the only one to express feelings." She didn't express it directly; she did so indirectly.
I guess this always bothered me. The first time I saw these episodes, I was like, WTF? When did she tell him she liked him that way? What did I miss?
